INSTRUCTIONS FOR FILLING OUT AND SUBMITTING
THE MILITARY AFFIDAVIT FOR JURY DUTY EXEMPTION
(Can be filled out on line or printed and then filled out)

1.		Open the Military Affidavit form.
2.		Fill out the top third of the form (from Birthdate down to Reason for Request).
3.		Print the form (if you haven't already).
4.		Sign and date the form (just under the Name field).
5.		Have your Commanding Officer fill out the next section on the form (statement MUST be filled out and signed by applicant's Commanding Officer).
7.		Mail or Fax form to the Court Administrator's office before next jury summons date.
