INSTRUCTIONS FOR FILLING OUT AND SUBMITTING
THE INABILITY TO COMPREHEND ENGLISH AFFIDAVIT FOR JURY DUTY EXEMPTION
(Can be filled out on line or printed and then filled out)

1.		Open the Inability To Comprehend English Affidavit form.
2.		Fill out the top part of the form (from Birthdate down to inability to comprehend or communicate in the English language check box).
3.		Print the form (if you haven't already).
4.		Have translator print name in blank provided and sign (statement MUST be filled out).
5.		Fill out the middle section of the form in front of Notary and have Notary sign form (affidavit MUST be notarized).
6.		Bring form to the Court Administrator's office on the 4th floor of the Nueces County Courthouse before next jury summons date.
