You must bring your entire jury summons with you on your START DATE. Be sure the removable protion of the jury summons attached to Part A is filled out and a contact phone number is supplied on the reverse side.

If you lose or forget your jury summons just tell the staff at the doors of the Central Jury Rooom and you will be directed to where you can pick up and fill out a blank jury form.

If you filled out your jury summons online through our eResponse system, you must still bring in the removable portion attached to Part A. It does not need to be filled out.
